Recently, interest has significantly increased regarding low-dose minoxidil after news articles appeared in NY Times entitled “An Old Medicine Grows New Hair…” and in GQ, “What Guys With Thinning Hair Should Know About Off-Label Hair Growth Prescriptions,” as well as on local and national broadcast TV news, such as WPLG Local 10 Miami Oral Minoxidil.

Exosomes are extracellular vesicles (EVs) of endosomal origin that range in size between 30 and 150 nanometers. Exosomes are secreted by a wide range of cells, since virtually all living cells utilize exosome-mediated communication. Exosomes carry cell-specific cargos of proteins, lipids, and nucleic acids that are selectively taken up by recipient cells.

Exosome technologies have been developing rapidly in recent years and substantial growth is expected for the market as they get integrated into the fields of liquid biopsy, precision medicine and regenerative medicine. In particular, cancer derived exosomes influence the invasive potential of cells by regulating angiogenesis, metastasis, and immunity, making them an extremely useful source of biomarkers for use in cancer detection, diagnosis, and therapeutic selection.

The cargo contained within exosomes can offer prognostic information for range of diseases—including cardiovascular, renal, neurodegenerative, and metabolic diseases—as well as cancer.  Researchers investigating exosome biomarkers have discovered, identified, and reported the presence of hundreds of biomolecules within the lumen of exosomes. This discovery has compelled a rapid rise in exosome-related cancer biomarker research, including the use of exosomes for the detection, monitoring, and treatment of a diverse range of oncologic conditions.

Importantly, exosomes are present within a diverse range of biofluids, including serum, plasma, urine, seminal fluid, CSF, saliva, tears and breast milk. For this reason, exosome-based diagnostics are minimally invasive, offering ease of use and speed of detection. Exosomes can also act as prognostic indicators and predictors of a patient’s response to a specific course of treatment.

Exosomes are also being explored for their use as cell-free therapeutics. For example, if a patient has a disease caused by a missing or defective protein or microRNA, the patient’s exosomes can be isolated, modified with the appropriate siRNA or protein, and injected back into the patient for treatment. Numerous approaches are being researched for creating drug-loaded exosomes and exosomes themselves can exert powerful effects. For example, mesenchymal stem cell (MSC) derived exosomes have the capacity to suppress inflammation, prevent scar tissue formation, and mediate a healthy immune response.

The Era of Exosomes

For these reasons, exosomes have gone from being overlooked to rapidly gaining momentum as a novel strategy for accessing the therapeutic effects of cells without the risks and difficulties of administering cells to patients. Although exosomes were discovered more than 30 years ago, it was not until recently that the scientific community began to give credit to exosomes for a range of promising traits.

When exosome publications are analyzed, U.S. authors have published approximately 40% of the articles and China comes in second place. When the term “exosome” is searched in the scientific literature, Dr. Susanne Gabrielsson from the Karolinska Institutet in Sweden is the leading author. When the term “extracellular vesicles” is searched, Unicyte AG, a regenerative medicine unit of Fresenius Medical Care, takes the lead. Headquartered in Switzerland, the company has published more than 100 papers on exosomes and extracellular vesicles in combination with Professor Giovanni Camussi.

To date, three pharma companies (Takeda, Jazz Pharmaceuticals, and Roche Pharmaceutical Company) have signed partnership deals with exosome companies offering payment terms structured to deliver at or around $1 billion dollars. Within the past 5 years, there has been at least 7 partnership deals within the exosome industry, 8 large venture capital events, and 2 landmark acquisitions.

Globally, there are at least 204 clinical trials focusing on exosome-related studies. Of these, 114 trials are evaluating exosome-based therapeutics and 74 trials are testing exosome-based diagnostic tests. Currently, 93 of the exosome trials (45%) are observational studies and 111 of them (55%) are interventional studies. The exosome industry has also achieved two approvals of FDA-approved diagnostic tests: Bio-Techne’s ExoDx Prostate IntelliScore Test for prostate cancer and Guardant’s 360 CDx test for non-small cell lung cancer.

What To Do When Natural Hair Growth Treatments Are Not Enough

Following are excerpts from a 20-minute “consult” without the detailed background information in which Wade seeks advice for his thinning hair. 
DR: “Tell me a little bit about your hair loss situation and maybe what you’re seeing in the mirror and how it looks to you when you look at your hair and when you first became aware of some hair changes?
WL: “Maybe in my late 20s I just started to notice like over here on the sides, particularly. It would just start to go back a little bit more. Nothing that was alarming, but something I do recall noticing it’s kind of like when you get your first wrinkle. A percentage of loss is happening, each year, and now I have very fine hair, as well as existing blonde and very light.”
WL: “So that also amplifies it in certain lighting when I’m on lights and things like that, so you know that that light shines right through my hair. I’d like it to be a little thicker so I don’t go full Kojak too soon.
DR: “Sure, and as mentioned before you do have some family hair loss on your dad’s side. So we know hair loss that is genetic male pattern baldness does come from the families, mom-side or dad-side or both and it can skip generations and siblings.”
DR: “The good news about that crown area is that we can definitely protect and restore that area, probably with non-invasive therapies. But if you’re looking to change the way your hairline looks then obviously we would need to do something more aggressive.”
WL: “Well, I think, if I had a magic wand, I would just kind of return it back to maybe where it was when I was in my early 20s. I’d have no receding parts here.”
DR: “Teenager hairline is not going to look normal and natural as you continue to age, and so we need to talk about an age-appropriate hairline, which is going to have a little bit of a recession to it.”
DR: “But the cool thing about this kind of technology is that even at four days out from the procedure you can do tennis and golf and lift weights, you can be at the gym sweating sauna. It takes about four to five months for those hairs to start to kick in and grow.”

or "Why Resolutions Are Like Stinky, Spoiled Milk"

Hello, it’s Dr. Bauman.

Lots of things have expiration dates:

Milk.

Vitamins.

Celebrity marriages. :)

And as it turns out, New Year’s resolutions have an expiration date, too. In fact, research conducted by Strava, a data analysis firm, found that January 19th is the day most people abandon their resolutions.

(Strava calls it “Quitter’s Day”. Ouch.)

So if you dropped your New Year’s resolutions like a carton of spoiled milk, you’re not the only one.

It happens to most people.

And one reason why is because, frankly, resolutions aren’t very effective.

Think about it. 

Most resolutions are well-intentioned but vague. 

For example, let’s say you set a resolution this year to get better hair health. The good intention is there, but what exactly does “better hair health” mean? It’s too vague to say, right?

As a result, there’s no way to measure your progress. Are you moving forward? Or running in place? Without a sense of forward momentum, it’s easy to wind up frustrated and confused.

No wonder so many people bail on their New Year’s resolutions after a couple of weeks.

But here’s good news:

By making a couple of simple tweaks, you can actually stick to your New Year’s resolutions -- and achieve any goal you set. Here’s how.

First, turn your resolution into a specific, process-oriented goal.

Instead of wishing and hoping for “better hair health”, you could turn that resolution into a specific goal like, “I want a visibly thicker, fuller head of hair in the next 9 to 12 months.”

Then, you follow a process that moves you towards this goal. For example: 

Eat foods rich in hair-healthy nutrients like biotin, vitamin E, and omega-3 fatty acids at each meal. Practice meditation 15 minutes per day to reduce stress. Meet with a qualified hair doctor to discuss your hair health challenges, goals, and treatment options.

If you stick to a winning process, results will follow.  

Second, actively measure your progress. 

Some goals are relatively easy to measure.

An Uncontrolled Case Series Using a Botanically Derived, β-Cyclodextrin Inclusion Complex in Two Androgenetic Alopecia-Affected Male Subjects

Abstract Drug-based monotherapy provides limited clinical benefits in polygenic disorders, such as androgenetic alopecia. Possible benefits must be measured against non-trivial risks of negative side effects. Several well-controlled, peer-reviewed, basic science studies have demonstrated novel mechanisms of action and potential utility for natural-based phytochemicals in the treatment of androgen-mediated disorders, including androgenetic alopecia. Yet, due to phytochemical instability, volatility, and incompatibility, the bridge from in vitro potential to clinical efficacy remains largely unmet. Recent advances in nanomaterial manipulation provide enhanced platforms, such as cyclodextrins, in which these phytochemicals may be enveloped and delivered without triggering the loss of intended function. Unexpected, positive results of an uncontrolled case series for a cyclodextrin-enabled, natural-based formula containing γ linolenic acid, β-Sitosterol, epigallocatechin gallate, and genistein, administered concomitantly via oral and topical form in two androgenetic alopecia-affected, male subjects over the course of 270 days were found. At baseline, significant baldness in the vertex scalp of both subjects was observed. Subsequent 90-day time points demonstrated marked hair thickening. Why We Shlepped to St. Louis Hair Transplant 360

Why did I schlep >1,300 miles with my hair transplant team?

If you were a top athlete or part of a high-performing athletic team, you’re always trying to improve your game mentally and physically. A surgeon and a surgical team are no different. But instead of hitting the gym, a performance camp, or “spring training,” where does a hair transplant team go to take a break from their days of surgery to “practice” and improve, acquire and share their wisdom and skills with their teammates and colleagues? 

They step out of the regular routine and attend a hands-on surgical workshop. Last month, the entire Bauman hair transplant team had a unique opportunity to attend “Hair Transplant 360” at St Louis University’s Center for Practical Anatomy & Clinical Experience (PACE), a hands-on cadaver workshop for both surgeons and their assistants organized by Dr. Sam Lam, Emina Vance and supported by the ISHRS International Society of Hair Restoration Surgery. The thirteen of us left South Florida and headed to St. Louis MO. Up early and back late—the bus left the hotel at 6:30am and returned 6:30pm—each day was packed with about 10hrs of CME Continuing Medical Education specifically for hair transplant surgeons and their assistants. Topics presented and skills practiced included basic techniques to the most advanced therapeutic interventions, and even those team members who have decades of experience with over 10,000 patients and millions of grafts placed, had an amazing time.

It was an honor to be invited to be on the faculty with my esteemed hair transplant colleagues from all across the country and working with attendees at all stages of their careers, but all with a keen interest in advancing their hair transplant skills and aesthetic eye.
